HIPAA Policies Policies

The University of Louisiana at Monroe’s HIPAA Documentation Policy states that all ULM health care facilities and providers must maintain documentation of patient privacy and protected Health Information in accordance with applicable regulations. The purpose of this policy is to ensure all ULM health care facilities and providers will adhere to all documentation requirements as stated in 45 C.F.R. 164.530(j) and other applicable federal, state, and/or local laws and regulations.

The HIPAA Minimum Necessary Information Policy defines the Minimum Necessary Rule under HIPAA and describes reasonable efforts to limit access to Protected Health Information based on the minimum information necessary.

The University of Louisiana at Monroe’s HIPAA Privacy Official and Complaint Contact Policy states that ULM must designate a HIPAA Privacy Official who will oversee and implement the privacy policies and procedures while ensuring ULM is in compliance with the HIPAA regulations.

ULM’s Patient’s Right to Access and Obtain a Copy of their Protected Health Information Policy states that all ULM health care facilities and providers must provide patients with a right of access to inspect and obtain a copy of their Protected Health Information except as otherwise statutorily excluded.

The ULM Patient’s Right to Request and Receive Confidential Communications Policy defines the duty of ULM providers and facilities to provide patients with mechanism to make reasonable requests to receive confidential communications by alternative means or at alternative locations of their Protected Health Information.

The HIPAA Privacy Training and Education Requirements for Member of ULM’s Workforce official policy describes the requirements for all ULM health care facilities and providers to provide members of its workforce with education and training on the ULM policies and procedures on Health Information Privacy and the HIPAA Privacy Regulations.

The University of Louisiana Monroe’s Use of PHI for Treatment, Payment and Healthcare Operations Official Policy establishes the requirements for using or disclosing Protected Health Information to carry out treatment, obtain payment, or conduct health care operations in compliance with HIPAA Privacy regulations.

The University of Louisiana at Monroe’s (ULM) Use and Disclosure of Protected Health Information that Requires an Individual’s Written Authorization Policy establishes the requirements for obtaining patient authorization for the disclosure of protected health information to family members, friends, or other person identified by the patient according to HIPAA and any other state or Federal regulations.

The University of Louisiana at Monroe’s Use or Disclosure of Protected Health Information to Persons Involved in Patient’s Care and for Notification Purposes Official Policy establishes the requirements for disclosing a patient’s protected health information to family members, friends, or any other person identified by the patient according to HIPAA regulations as well as any other state or Federal Law.
